February 19, 2024 REVENUE MEMORANDUM CIRCULAR NO. 32-2024 SUBJECT : Lost Twenty-Eight (28) Sets of Unused/Unissued BIR Form No. 0535 Taxpayer Information Sheet TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of twenty-eight (28) sets of unused/unissued BIR Form No. 0535 Taxpayer Information Sheet, bearing the following serial numbers: Serial Number Quantity TIS201900273705 1 set TIS202000140462-TIS202000140467 6 sets TIS202000141601-TIS202000141615 15 sets TIS202000141620-TIS202000141625 6 sets The abovementioned forms were reported as lost by Mr. Asgar C. Macabanding, Revenue Officer II (A), Revenue District Office No. 41, Revenue Region No. 7B, East NCR, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
