May 29, 1974 REVENUE MEMORANDUM CIRCULAR NO. 31-74 TO : All Internal Revenue Officers, Employees, Provincial/City Treasurers and others concerned SUBJECT : Loss of two (2) pads or 100 pieces of Residence Tax Certificates A (BIR Form 9.01) bearing Serial Numbers 8454051-8455100, and 8457001-8457050, inclusive Serial Numbers Quantity 8454051-8455100 one (1) pad or fifty (50) pcs. 8457001-8457050 one (1) pad or fifty (50) pcs. The above-mentioned Residence Tax Certificates Class A (BIR Form 9.01) were reported to the Commissioner of Internal Revenue by TOM PANGILAMEN, City Treasurer (Officer-In-Charge) of the province of Maguindanao to have been lost, presumably in transit from Manila to his office. The two (2) pads were a part of the one thousand (1,000) pads personally taken by the authorized representative of the office of the Treasurer of the Province of Maguindanao at the Accountable Forms Division, who also undertook the shipment of the same. However, the boxes containing the residence tax certificates were allegedly received by them in bad order. Internal revenue officers, employees, Provincial/City Treasurers and others concerned are requested to promptly notify this office in case the above-mentioned Residence Tax Certificates are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
