May 3, 1979 REVENUE MEMORANDUM CIRCULAR NO. 30-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of Two (2) Sets of Revenue Tax Receipts (CB Form No. CB RCO 02.02) bearing Serial Nos. 0156372 and 0156373 Notice is hereby given of the loss of two (2) sets of Revenue Tax Receipts, to wit: aisa dc Serial No. Quantity 0156372 One (1) Set 0156373 One (1) Set The above Revenue Tax Receipts were reported to the Regional Officer-In-Charge, Revenue Region No. 8, Tacloban City, by SEVERINO C. ZALAVARRIA, Revenue Cash Clerk, Office of the Collection Agent, as missing or lost sometime on January 17, 1979. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
