May 28, 1974 REVENUE MEMORANDUM CIRCULAR NO. 30-74 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of Thirty-Three (33) Pieces of Revenue Official Receipts (BIR Form 25.24) bearing Serial Numbers 1402117 to 1402150, Inclusive Serial Numbers Quantity 1402117-1402150 thirty-three (33) pieces The above-mentioned Revenue Official Receipts were reported to the Commissioner of Internal Revenue by Revenue Collection Agent SANTOS S. TAMPOS of Valencia, Bohol to have been lost on May 15, 1974 in the vicinity of the LTC Office of Tagbilaran City. That while he was helping the LTC clerk stenciling his motorcycle which he uses in the course of his official duties as collection agent, he placed his brown portfolio on top of the hood of a parked jeep, when he and the LTC clerk were through with the stenciling of his motorcycle, the parked jeep was no longer there, so was his brown portfolio which contained the official receipts and some of his papers. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Official Receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
