June 21, 2005 REVENUE MEMORANDUM CIRCULAR NO. 28-05 SUBJECT : Lost Two (2) Pads of Unissued BIR Form No. 2524 Revenue Official Receipt TO : All Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of two (2) pads of unissued BIR Form No. 2524 Revenue Official Receipt (ROR) bearing the following serial numbers: ROR200500466551-ROR200500466650 The abovementioned forms were reported as lost by Mr. Oliver R. Pador, Supply Officer II, Revenue Region No. 01, Calasiao, Pangasinan, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. HTCSDE Internal Revenue Officials, employees and others are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
