April 28, 2004 REVENUE MEMORANDUM CIRCULAR NO. 27-04 SUBJECT : Missing Two (2) original copy of unused BIR Form No. 1954 Certificates Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing two (2) original copy of unused Certificates Authorizing Registration (BIR Form No. 1954) with the following serial numbers: SCADIT CAR200400021976 CAR200400021978 The abovementioned forms were reported as missing by MS. NANETTE M. CANILLADA, of BIR-Finance Division, Revenue Region No. 09, San Pablo City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
