April 12, 2006 REVENUE MEMORANDUM CIRCULAR NO. 25-06 SUBJECT : Lost two (2) used and two (2) unused sets of BIR Form No. 1954 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of two (2) used and two (2) unused sets of BIR Form No. 1954 Certificate Authorizing Registration (CAR) with the following serial numbers: USED UNUSED CAR200300156260 CAR200500005695 CAR200400180650 CAR200400317850 The abovementioned forms were reported as lost by Ms. Virgilia A. Aguirre, Ms. Anabel S. Evangelista, and Ms. Vivenciana G. Faustino, Revenue Officers assigned at Revenue District Office No. 25, Revenue Region No. 05, Valenzuela City, Consequently, said lost forms have been cancelled and all official transactions involving the use thereof are considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same. caSDCA (SGD.)
