April 19, 1979 REVENUE MEMORANDUM CIRCULAR NO. 24-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of Two (2) Sets of Revenue Tax Receipts (CB Form No. CB RCO 02.02) bearing Serial No. 1049149, 1049150, and Three (3) Copies of RTR No. 1049148 Notice is hereby given of the loss of the following Revenue Tax Receipts; Serial Nos. Quantity 1049149 One (1) set 1049150 One (1) set 1049148 Three (3) copies (Taxpayer's copy, BIR file copy, DPC file copy) The two (2) sets and three (3) copies of the above-mentioned Revenue Tax Receipts were reported as lost, by MRS. NELLIE SUMAGAYSAY, Revenue Collector I, Revenue District No. 4, Revenue Region No. 3-B, Valenzuela, Metro Manila, to the Regional Director, same Revenue Region. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof. aisa dc EFREN I.
