June 14, 2005 REVENUE MEMORANDUM CIRCULAR NO. 23-05 SUBJECT : Lost Five (5) Sets of Unissued BIR Form No. 0423 Apprehension Slip TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of five (5) sets of unissued BIR Form No. 0423 Apprehension Slip (APS) bearing the following numbers: APS200300026583-APS200300026587 The abovementioned forms were reported as lost by Ms. Dirce M. Diaz, Revenue Officer, Revenue Region No. 07, Quezon City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. ACHEaI Internal Revenue Officials, employees and others are requested to notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
