April 22, 1976 REVENUE MEMORANDUM CIRCULAR NO. 21-76 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One (1) Pad of Unissued Residence Certificate C and One (1) Pad of Residence Certificate C-1, s. 1976 Notice is hereby given of the loss of one (1) pad each of Residence Certificates C and C-1, s. 1976, to wit: Kind Serial Number Quantity 1. Residence Certificate C 11601 11625 Twenty-five (25) sets 2. Residence Certificate C-1 8376-8400 Twenty-five (25) sets The above-mentioned Residence Certificates were reported to have been lost by Revenue Examiner BENJAMIN R. SAMOY, one of the revenue examiners assigned to assist income tax filers on April 14, 1976, at the Marble Hall, Finance Building, Manila. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case any of the above-mentioned Residence Certificates is found, taking necessary steps in the meantime to prevent the improper use or fraudulent disposition thereof.
