May 29, 1975 REVENUE MEMORANDUM CIRCULAR NO. 21-75 TO : All Internal Revenue Officers, Employees and Others Concerned SUBJECT : Loss of One (1) Pad of Fifty (50) Sets of Revenue Official Receipts (BIR Form 25.24) Bearing Serial Numbers 3700451-3700500 Inclusive SERIAL NUMBER QUANTITY 3700451-3700500 Fifty (50) sets The above Revenue Official Receipts were reported lost by Mr. Jacob B. Bringas, Revenue Collection Agent of Cuyapo, Nueva Ecija on February 24, 1975, and that thirty (30) sets of which bearing Serial Numbers 3700451-3700480 were already issued, while twenty (20) sets bearing Serial Numbers 3700481-3700500 are still unused. Internal Revenue Officers, Employees and Others Concerned are requested to promptly notify this Office in case the abovementioned Revenue Official Receipts are found, taking necessary steps in the meantime, to prevent the improper and fraudulent disposition or use thereof.
