March 26, 1974 REVENUE MEMORANDUM CIRCULAR NO. 21-74 TO : All Internal Revenue Officers, Employees, and others concerned SUBJECT : Loss of One (1) Piece of Central Bank Official Receipt (Form BIR-2), Bearing Serial No. 188950 Notice is hereby given of the loss of one (1) piece of Central Bank official receipt, to wit: Serial No. Quantity 188950 one (1) piece The above-mentioned Central Bank official receipt was discovered missing by Mr. ARTURO F. GOTARDO Pro Manager of Philippine Commercial and Industrial Bank, Cadiz Branch, Cadiz City last February 2, 1974. Internal revenue officers, employees, and others concerned are requested to promptly notify this office in case the abovementioned Central Bank official receipt is found and presented as evidence of payment of internal revenue taxes.
