XENIA S. AYOP (vendor) to wit: Serial Number Quantity TVN199900149288 One (1) set The abovementioned form which was reported as lost by MS. MARIA JESSICA GODINEZ-LUCENTE, Computer Operator I and in-charge of the preparation of BIR Form No. 0421-Tax Verification Notice of Revenue District Office No. 115, Digos, Davao Del Sur, has been cancelled and all official transactions involving the use thereof are therefore invalidated. Internal Revenue Officers, employees and others concerned are requested to promptly notify this Office in case the abovementioned copies are found, taking the necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
