March 19, 1976 REVENUE MEMORANDUM CIRCULAR NO. 17-76 SUBJECT : Loss of eighteen (18) sets of unissued Revenue Official Receipts (BIR Form 25.24), bearing Serial Number 0087833-0087850, inclusive TO : All Internal Revenue Officers, Employees, and others concerned Notice is hereby given of the loss of eighteen (18) sets of Revenue Official Receipts (BIR Form 25.24), to wit: Serial Number Quantity 0087833 to 0087850 Eighteen (18) Sets The Regional Director of Butuan City, Revenue Region No. 10-B, reported to this Office, thru the Revenue Service Chief (Collection), the loss of one (1) pad of Revenue Official Receipts (BIR Form 25.24) by GUILLERMO P. QUINLAT of Sta. Josefa, Agusan del Sur, Municipal Treasurer and Acting Revenue Collection Agent thereat. In his affidavit, Mr. Quinlat stated that in the preparation of his monthly report as of August 31, 1974, the said booklet was found missing and that the same was partially issued, with only eighteen (18) sets left on the stub. Internal revenue officers and others concerned are requested to promptly notify this Office in case the above-mentioned receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
