June 28, 1975 REVENUE MEMORANDUM CIRCULAR NO. 17-75 SUBJECT : Proof of Personal Qualifications Stated in the Personal Data Sheets (Information Sheet) It has come to the knowledge of this Office that qualifications claimed by many employees of this Bureau in the personal data sheets which were the basis for their promotions or reappointments are not accurate. False statements on the personal data sheet which is under oath makes the employee concerned liable for perjury and falsification of public documents. If proven, the penalty for these offenses is dismissal from the service. The personal data sheets which were submitted in connection with appointments to positions in the Staffing pattern of this Bureau as reorganized, shall be verified against the personal records (201 file) in the Personnel Division. Those whose proof of claimed qualifications are not yet on file in the records of the Personnel Division are hereby required to submit to the said division proof of their qualifications (report of rating of civil service eligibilities, diplomas or transcript of school records) not later than August 31, 1975. Those who do not show proof of their qualifications will be investigated and the appropriate administrative charges shall be filed against them, if warranted.
