May 16, 1977 REVENUE MEMORANDUM CIRCULAR NO. 15-77 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of 2,400 Sets of Auxiliary Invoices for Timber (BIR Form No. 14.04) bearing Serial Nos. 004561 to 006960 and 400 sets of Auxiliary Invoices for Minor Forest Products (BIR Form No. 14.05) bearing Serial Nos. 000481 to 000880 Serial Number Quantity Auxiliary Invoice (BIR Form No. 14.04) Nos. 004561 to 2,400 sets (60 pads) 006960 Auxiliary Invoice (BIR Form No. 14.05) Nos. 000481 to 400 sets (10 pads) 000880 The loss of the above-mentioned internal revenue auxiliary invoices was reported by the Chief of the Accountable Forms Division to this Office based on a report submitted by Mr. Felix Velasco, Jr., Acting Chief of the Miscellaneous Forms Section, who found the said invoices missing after conducting an inventory of all miscellaneous forms of the Accountable Forms Division that were transferred from the Finance Building to the new BIR National Office Building in Quezon City. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the abovementioned Auxiliary Invoices are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof. aisa dc EFREN I.
