March 21, 1968 REVENUE MEMORANDUM CIRCULAR NO. 14-68 SUBJECT : Publishing Memorandum-Circular No. 3-68 of the Department of Finance dated March 11, 1968 TO : All Internal Revenue Officers and Others Concerned For the information and guidance of all internal revenue officers and others concerned there is quoted hereinbelow Memorandum-Circular No. 3-68 of the Department of Finance dated March 11, 1968: "MEMORANDUM-CIRCULAR NO. 3-68 "TO : All Heads of Bureaus and Offices Under the Department of Finance and Offices of Provincial/City Treasurers/assessors "SUBJECT : Notice of loss of 5 pads General Form No. 13-A Official Receipts "Quoted hereunder in full for your information and guidance is a letter of the Director of Printing dated March 7, 1968 bearing on the above subject-matter: 'Please be informed that after a thorough and careful verification of the General Form No. 13-A, Official Receipts, now in process of printing, it has been found that five (5) pads thereof bearing Serial Nos. H-7048351 to H-7048400; H-7048451 to H-7048500; H-7048601 to H-7048650; H-9770151 to 9770200; and R-9770351 to H-9770400 were missing. Incident thereto, this office has ordered an immediate investigation of the case and a careful study of the procedures practiced in the handling of the said accountable form be made for remedial measures to institute full-proof security thereon. 'Cognizance of the grave consequence that may arise should these missing pads fall in unscrupulous hands, this matter is being reported so that appropriate steps can be taken and all concerned be notified hereof to preclude its anomalous use which will deprive our government of its lawful income."
