March 4, 2004 REVENUE MEMORANDUM CIRCULAR NO. 14-04 SUBJECT : Cancellation of One (1) Set of Unused BIR Form No. 1954 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the cancellation of one (1) set of an unused Certificate Authorizing Registration (BIR Form No. 1954) with the hereunder stated serial number, to wit: CAR200300225572 The abovementioned form was reported as defective by MS. NANETTE M. CANILLADA, of BIR-Finance Division, Revenue Region No. 09, San Pablo City, and has consequently been cancelled. All official transactions involving the use of said form is therefore considered as INVALID.
