December 10, 2020 REVENUE MEMORANDUM CIRCULAR NO. 135-20 SUBJECT : Lost Six (6) Sets of Unused/Unissued BIR Form No. 2298 Product Replenishment Certificate TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of six (6) sets of unused/unissued BIR Form No. 2298 Product Replenishment Certificate, bearing the following serial numbers: PRC201600002094 PRC201600002099 The abovementioned forms were reported as lost by Ms. Angelie Monica H. Asuncion, Revenue Officer I, Export Permit and Product Replenishment Section, Excise Large Taxpayers Field Operations Division, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. HTcADC Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
