March 2, 1972 REVENUE MEMORANDUM CIRCULAR NO. 13-72 SUBJECT : Loss of One (1) Official Receipt (BIR Form No. 25.24) No. 5040351 (Original, Duplicate, Triplicate and Quadruplicate Copies) TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of one (1) official receipt (BIR Form No. 25.24) No. 5040351 (Original, duplicate, triplicate and quadruplicate copies). The above official receipt was discovered lost by the Chief, Administrative Branch, JOSE G. EALDANA, Revenue Region No. 14, Bacolod City. The loss was reported to the Revenue Regional Director, Revenue Region No. 14, Bacolod City and to this Office, on the 1st Indorsement of the Regional Director, dated January 6, 1972, original with enclosure attached. Internal Revenue Officers, Employees and Others Concerned are requested to promptly notify this Office in case the above-mentioned official receipt (BIR Form No. 25.24), in found, taking necessary steps in the meantime, to prevent improper or fraudulent disposition or use thereof.
