April 5, 2005 REVENUE MEMORANDUM CIRCULAR NO. 13-05 SUBJECT : Loss of the Duplicate to Quintuplicate Copies from Six (6) BIR Form No. 2524 Revenue Official Receipt and Another Forty-four (44) Sets of the Same BIR Accountable Form TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the duplicate to quintuplicate copies from six (6) issued BIR Form No. 2524 Revenue Official Receipt (ROR) bearing serial nos. ROR200400010151 to ROR200400010156, and another forty-four (44) unissued sets of the same BIR accountable form bearing the following serial numbers: ROR200400010157-ROR200400010200-UNISSUED The abovementioned forms were reported as lost by Ms. Julie Aguilos-Dellota, Revenue Officer I, Revenue District Office No. 72, Roxas City. The forty-four sets of unissued ROR have consequently been cancelled and all official transactions involving the use of said forms are therefore considered as INVALID. STADIH Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
