February 19, 1979 REVENUE MEMORANDUM CIRCULAR NO. 12-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One (1) Original Copy of Revenue Tax Receipt (CB Form No. CB RCO 02.02) bearing Serial No. 0795776 Notice is hereby given of the loss of the original copy of Revenue Tax Receipt, to wit: Serial No. Quantity 0795776 One (1) Piece The original copy of the above Revenue Tax Receipt was reported to the Chief, Accountable Forms Division by ESTRELLITA T. RONAN, Acting Accountable Forms Custodian, Revenue Region No. IV-B, Quezon City, as missing from the booklet.
