April 15, 1977 REVENUE MEMORANDUM CIRCULAR NO. 12-77 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One Pad Official Receipts (BIR Form No. 25.24) bearing Serial Numbers 5673351-5673400 Serial Number Quantity 5673351 to 5673378 Twenty-eight (28) pieces already issued 5673379 to 5673400 Twenty-two (22) pieces still unissued The above Revenue Official Receipts were reported lost by Regional Director Alfonso B. Camillo of Revenue Region No. 10-A, Cagayan de Oro City, by a radiogram to Assistant Commissioner Lauro D. Abrahan based on the report of the Collection Agent of Butig, Lanao del Sur. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Official Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
