March 4, 1964 REVENUE MEMORANDUM CIRCULAR NO. 12-64 SUBJECT : Loss of Revenue Official Receipts (BIR Form No. 25.24) and 1963 Residence Tax Certificates Classes "B", "C" and "C-1" TO : All Revenue Officers and Employees and Others Concerned Notice is hereby given of the loss of revenue official receipts (BIR Form No. 25.24) and 1963 Residence Tax Certificates classes "B", "C" and "C-1", to wit: Serial Numbers Quantity R OR 1776913 - 1776950 38 receipts R. C. "B" 0443551 - 0442650 100 receipts R. C. "C" 6726 - 6750 25 receipts R. C. "C-1" 6451 - 6475 25 receipts The above revenue official receipts (BIR Form No. 25.24) were lost by Collection Agent Filomeno B. Balbin while he was on his way from his office at Candon, Ilocos Sur, to his second station town Banayoyo, same province, on April 25, 1963; while the above Residence Tax Certificates classes "B", "C" and "C-1" were lost by Collection Agent Jose Puntanilla on a Philippine Rabbit bus which he took from Manila to his station town San Esteban, Ilocos Sur, on February 15, 1963. Internal Revenue Officers and others concerned should promptly notify this Office in case any of those Residence Tax Certificates is found taking necessary steps in the meantime to prevent improper or fraudulent disposition or use thereof.
