February 19, 1979 REVENUE MEMORANDUM CIRCULAR NO. 11-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One (1) Central Bank Copy of Revenue Tax Receipt (CB Form No. CB RCO 02.02) bearing Serial No. 0500948 Notice is hereby given of the loss of the Central Bank Copy of Revenue Tax Receipt, to wit: cdt Serial No. Quantity 0500948 One (1) Piece The Central Bank copy of the above Revenue Tax Receipt was reported to the Regional Director, Revenue Region No. 2, Tuguegarao, Cagayan by ELVIRA A. CADIENTE, Revenue Collection Agent of Bayombong Nueva Vizcaya, as missing from the booklet. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipt is found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
