JOSE CAROLINO & MR. AMADO LANDICHO to wit: Serial Number Quantity TVN 199900227693 One (1) set TVN 199900227694 One (1) set The abovementioned forms which were reported as lost by MR. CRISPIN P. TENORIO, in-charge of the issuance of Tax Verification Notice of Revenue District Office No. 59, Lipa City, have been cancelled and all official transactions involving the use thereof are therefore invalidated.
