December 12, 1978 REVENUE MEMORANDUM CIRCULAR NO. 109-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of One (1) Set of Revenue Tax Receipt (CB RCO Form No. 02.02) Bearing Serial Number 350326 Notice is hereby given of the loss of one (1) set of Revenue Tax Receipt, to wit: Serial Number Quantity 350326 One (1) Set The above Revenue Tax Receipt was reported as missing or lost by the Regional Director, Revenue Region No. 6-B, Bacolod City in his report on RTR accountability. Internal Revenue Officers, Employees, and Others Concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipt is found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof. cdt CONRADO P.
