December 4, 1978 REVENUE MEMORANDUM CIRCULAR NO. 098-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of One (1) Pad of Revenue Official Receipts (BIR Form No. 25.24) Bearing Serial Numbers 0978951-0979000 J, Inclusive Notice is hereby given of the loss of one (1) pad of Revenue Official Receipts, to wit: Serial Number Quantity 0978951-0978982 J (Issued) Thirty-two (32) pieces 0978983-0979000 J (Unissued) Eighteen (18) sets The above Revenue Official Receipts were alleged to have been burned on July 16, 1978 during the fire at Maco, Davao that gutted the building housing the Office of the Collection Agent, as reported by EUFEMIA P. CHIONG, Acting Collection Agent, Maco Davao to the Revenue District Office, Tagum, Davao. Internal Revenue Officers, Employees, and Others Concerned are requested to promptly notify this Office in case the above-mentioned Revenue Official Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof. cdt CONRADO P.
