September 1, 2016 REVENUE MEMORANDUM CIRCULAR NO. 094-16 SUBJECT : Loss of Several Product Replenishment Certificate BIR Form No. 2298 TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: Accountable Form Serial Number Missing Copy/Set BIR Form No. 2298 PRC201200002935 duplicate copy (Product Replenishment Certificate) (used/filled-up) BIR Form No. 2298 PRC201200003895 triplicate copy (Product Replenishment Certificate) (used/filled-up) BIR Form No. 2298 PRC201200004891 triplicate copy (Product Replenishment Certificate) (used/filled-up) The abovementioned forms were reported as lost by Mr. Renato M. Garcia, Revenue Officer II, Excise Large Taxpayers Field and Operations Division and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
