November 18, 2010 REVENUE MEMORANDUM CIRCULAR NO. 089-10 SUBJECT : Loss of Forty (40) Unused Sets of BIR Form No. 0535 Taxpayer Information Sheet TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: cIADTC Accountable Form Serial Number/s Quantity BIR Form No. 0535 TIS200600042262- 39 sets (Taxpayer Information Sheet) TIS200600042300 BIR Form No. 0535 TIS200600042257 1 set (Taxpayer Information Sheet) The loss of the said accountable forms was reported by Mr. Mario M. Pagsuyuin, Revenue Officer I, Revenue District No. 60, Revenue Region 9, San Pablo City. Consequently, the same accountable forms have been cancelled and all official transactions involving the use thereof are considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
