December 7, 2012 REVENUE MEMORANDUM CIRCULAR NO. 087-12 SUBJECT : Damaged Accountable Forms TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the damaged accountable forms hereunder listed together with other pertinent information, to wit: aEcHCD Accountable Form Serial Number Damaged Set BIR Form No. 2524 ROR201004829825- 226 sets (unused) (Revenue Official Receipt) ROR201004830050 Accountable Form No. 51 ACF200800297095- 6 sets (unused) (Official Receipt) ACF200800297100 Said accountable forms were damaged during a flashflood, which were reported by Ms. Elsa C. Obungen, Revenue Collection Officer, Revenue District Office No. 98, Revenue Region No. 16, Cagayan de Oro City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to take the necessary measures to prevent the improper or fraudulent disposition or use of the same, and to promptly notify this Office of any such event.
