August 7, 2019 REVENUE MEMORANDUM CIRCULAR NO. 084-19 SUBJECT : Loss of Several Apprehension Slip BIR Form No. 0423 TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: APS200100018051-APS200100018100 1 pad (unused) APS200100018102-APS200100018150 49 sets (unused) The abovementioned forms were reported as lost by Ms. Emerita A. Teleron, formerly assigned as the OIC-Chief, Administrative Section, Revenue District Office No. 114, Revenue Region No. 19, Davao City and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
