Notice is hereby given of the loss of two (2) booklets of Residence Certificate Class "A", 1972 (BIR Form No. 9.01), to wit: SERIAL NUMBERS QUANTITY 274101 to 274150 One Pad 274151 to 274200 One Pad The above Residence Certificates were lost on January 15, 1972. The loss was reported by ATTY. JOSE C. CAPISTRANO, legal counsel of the Cathay Drug Co., Inc. in his affidavit dated January 18, 1972, attached copy submitted to this Office. Internal Revenue Officers, Employees and Others Concerned are requested to promptly notify this Office in case any of the abovementioned Residence Certificate is found, taking necessary steps in the meantime, to prevent improper or fraudulent use and disposition. cdt (SGD.)
