February 16, 2004 REVENUE MEMORANDUM CIRCULAR NO. 08-04 SUBJECT Missing One (1) Triplicate Copy of Unused BIR Form No. 1954 Certificate Authorizing Registration TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing one (1) triplicate copy of an unused Certificate Authorizing Registration (BIR Form No. 1954) with the hereunder stated serial number, to wit: CAR200300222435 The abovementioned form was reported as missing by MR. JUAN T. TRONO, assigned at the Office of the Assistant Regional Director, BIR-Revenue Region No. 06, Manila, and has consequently been cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
