February 11, 2002 REVENUE MEMORANDUM CIRCULAR NO. 08-02 SUBJECT : Loss of Three (3) Sets of Used BIR Form No. 0421 Tax Verification Notice TO : All Internal Revenue Officers, Employees and Others Concerned Notice is hereby given of the loss of THREE (3) SETS of used BIR Form No. 0421 Tax Verification Notice with following Serial Number: TVN200000156132 TVN200000156134 The abovementioned Forms were reported as lost by MS. RIZALYN S. AQUINO, of BIR-Revenue District Office No. 1 Laoag City Ilocos Norte, and have therefore been cancelled. All official transactions involving the use of such Forms are therefore considered as INVALID. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this Office, in the event that the aforesaid Forms are found; and to take the necessary measures to prevent the improper or fraudulent disposition or use of said Forms.
