September 3, 2010 REVENUE MEMORANDUM CIRCULAR NO. 073-10 SUBJECT : Loss of Several BIR Form No. 19.14 (Apprehension Receipt) TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: STcEaI Accountable Form Serial Number Missing Copy/Set BIR Form No. 19.14 285114 Duplicate copy (unused) (Apprehension Receipt) BIR Form No. 19.14 285601-285604 Original & Duplicate copies from (Apprehension Receipt) 4 sets (unused) BIR Form No. 19.14 285608-285612 Original & Duplicate copies from (Apprehension Receipt) 5 sets (unused) BIR Form No. 19.14 285201-285600 400 sets (unused) (Apprehension Receipt) BIR Form No. 19.14 285701-286000 300 sets (unused) (Apprehension Receipt) The loss of the said accountable forms was reported by Mr. Antonio P. Canono, Jr., Special Investigator I, National Investigation Division, BIR-NOB. Consequently, the same accountable forms have been cancelled and all official transactions involving the use thereof are considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
