September 1, 1978 REVENUE MEMORANDUM CIRCULAR NO. 067-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of Two (2) Pads and Two (2) Sets of Revenue Tax Receipts (CB RCO 02.02) Bearing Serial Numbers 1413526-1413550, Inclusive; 1413476-1413477; and 3321276-3321300, Inclusive Notice is hereby given of the loss of two (2) pads and two (2) sets of Revenue Tax Receipts, to wit: aisa dc Serial Number Quantity 1413526-1413550 One (1) pad 3321276-3321300 One (1) pad 1413476-1413477 Two (2) sets The above Revenue Tax Receipts were reported missing to the Officer-in-Charge, Assessment Branch, Revenue Region No. 4-A, Manila by JOSEFINA L. PUYO and TEOFILA M. GUEVARRA, both clerks of the same Office, on August 28, 1978. Internal Revenue Officers, Employees, and Others Concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
