August 14, 1978 REVENUE MEMORANDUM CIRCULAR NO. 060-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of One (1) Pad of Revenue Official Receipt (BIR Form No. 25.24) Bearing Serial Numbers 5666851-C to 5666900-C, Inclusive Notice is hereby given of the loss of one (1) pad of Revenue Official Receipt, to wit: Serial Number Quantity 5666851-C to 5666900-C One (1) Pad The above Revenue Official Receipt, an accountability of JOSE QUIROS, Revenue Collecting Agent, was discovered missing by Elma M. Gabica, COA Regional Auditor, San Fernando, La Union, in the course of an audit of the BIR Collection Agent's Office in San Fabian, Pangasinan. Internal Revenue Officers, Employees, and Others Concerned are requested to promptly notify this Office in case the above-mentioned Revenue Official Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof. aisa dc EFREN I.
