January 24, 1972 REVENUE MEMORANDUM CIRCULAR NO. 06-72 SUBJECT : Loss of Three (3) Original Copies of Residence Certificate Class B 19_ Numbers 884424, 884433 and 884436 and One (1) Duplicate Copy of Residence Certificate Class B 19_ No. 884482 (BIR Form No. 9.02) TO : All Revenue Officers, Employees and others Concerned Notice is hereby given of the loss of three (3) original copies and one (1) duplicate copy of Residence Certificate Class B 19_ (BIR Form No. 9.02), to wit: SERIAL NUMBERS QUANTITY 884424 One (1) receipt original copy. 884433 One (1) receipt original copy. 884436 One (1) receipt original copy. 884482 One (1) receipt duplicate copy. The above Residence Certificates were discovered lost by Collection Agent NENA C. FOGATA, Office of the Collection Agent, San Narciso, Zambales. The loss was reported to the Provincial Auditor in her letter dated January 10, 1972 attached copy submitted to this Office. Internal Revenue Officers, employees and Others concerned are requested to promptly notify this Office in case any of the abovementioned Residence Certificate is found, taking necessary steps in the meantime, to prevent improper or fraudulent use and disposition. aisa dc MISAEL P.
