August 14, 1978 REVENUE MEMORANDUM CIRCULAR NO. 058-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of One (1) Pad of Revenue Tax Receipt (CB RCO Form No. 02-02) Bearing Serial Numbers 15606753 to 15607004, Inclusive Notice is hereby given of the loss of One (1) pad of Revenue Tax Receipt, to wit: Serial Number Quantity 15606753 to 15607004 One (1) pad The above Revenue Tax Receipts, an accountability of SOLOMON A. AMBROSIO, Acting Revenue Collector of Bansalan, Davao del Sur, were reported missing by Director Abdon M. Mendigorin, RR II-B, Davao City. Internal Revenue Officers, Employees, and others Concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipts are found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
