June 21, 2018 REVENUE MEMORANDUM CIRCULAR NO. 057-18 SUBJECT : Lost Ten (10) Sets of Unused BIR Form No. 0423 Apprehension Slip TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of ten (10) sets of unused BIR Form No. 0423 Apprehension Slip, bearing the following serial numbers: APS200100000628 1 Set APS200100000638-APS200100000639 2 Sets APS200100000641-APS200100000647 7 Sets The abovementioned forms were reported as lost by Mr. Alfredo O. Pagdilao, Jr., Revenue Officer IV-A, Revenue District Office No. 27, Revenue Region No. 5, Caloocan City and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
