May 3, 2019 REVENUE MEMORANDUM CIRCULAR NO. 053-19 SUBJECT : Loss of Several Accountable Forms TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of the accountable forms hereunder listed together with other pertinent information, to wit: AHDacC Accountable Form Serial Number Quantity Documentary Stamps (P15.00 denomination) DSJ201605691501-DSJ201605691800 300 (unused/unissued) BIR Form No. 2524 (Revenue Official Receipt) ROR201301821511-ROR201301821550 40 sets (unused/unissued) BIR Form No. 2524 (Revenue Official Receipt) ROR201301821460-ROR201301821500 41 sets (unused/unissued) The abovementioned forms were lost during Marawi siege in Marawi City, as reported by Ms. Norhata M. Ibrahim, Revenue Officer I-Collection Agent, Revenue District Office No. 102, Revenue Region No. 16, Cagayan de Oro City and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
