July 29, 2008 REVENUE MEMORANDUM CIRCULAR NO. 053-08 SUBJECT : Lost One (1) Pad of an Unused Accountable Form No. 51 (AF51) TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of one (1) pad of an unused Accountable Form No. 51 (AF51) with the following serial numbers: ACF200700245501-ACF200700245550 The abovementioned accountable forms were reported as lost by Ms. Ranulfa Noel Victores, Revenue Officer III, Revenue District Office No. 33, Revenue Region No. 06, Manila, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. AIHDcC Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
