October 10, 2011 REVENUE MEMORANDUM CIRCULAR NO. 050-11 SUBJECT : Missing Two (2) Unused Sets of Accountable Form No. 51 Official Receipt TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing two (2) unused sets of Accountable Form No. 51 Official Receipt, bearing the following serial numbers: ACF201000377539 ACF201000377974 The abovementioned forms were reported as missing by Ms. Linda L. Maggay, Revenue Officer IV, Revenue District Office No. 52, Revenue Region No. 8, Makati City. Consequently, the same accountable forms have been cancelled and all official transactions involving the use thereof are considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
