February 14, 1964 REVENUE MEMORANDUM CIRCULAR NO. 05-64 TO : All Department Heads, Division Chiefs, Regional Directors, Chief Revenue Officers and Others Concerned SUBJECT : Submission to the Government Service Insurance System of Service Records of Government Employees For the information, guidance and compliance of all concerned, there is quoted below the contents of Circular No. 115, dated September 12, 1963 of the Government Service Insurance System: Attention is invited to the following provisions of Republic Act 3544 which was approved on June 21, 1963. LLpr "Sec. 13-A. Reconstruction of service record . For the purpose of section thirteen of this Act, such employee shall reconstruct and the System shall verify the record of his service rendered prior to June sixteen, nineteen hundred fifty-one, in accordance with such procedures as the Board may adopt and the provisions of the rules of Court. Such records of service shall be submitted to the system within two years from the date of approval of this amendatory Act, or from his re-employment if re-employed after said date, together with the certificates, affidavits, and other documents relied upon for reconstruction of said record. Any employee who fails to submit such record of service within said specified period shall lose automatically all rights granted by this Act pertaining to said service. Upon proper cause shown, the Board may grant extension of time for the presentation of said record of service or proofs thereof, but in no case shall the total extended period exceed one year from the expiration of the original period. Any certified copy of document or paper that may be necessary in the reconstitution of a record of service shall be furnished, free of charge, by an office instrumentality, or corporation of the Government." (Emphasis supplied) Pursuant to the above law, the System would like to emphasize the following: (1) Each employee, regardless of the status of his appointment or employment, shall reconstruct the records of his service in the government rendered prior to June 16, 1961. (2) Such reconstructed record of service shall be submitted to the System on or before June 21, 1965 deadline set. (3) Any employee who fails to submit to the System on or before June 21, 1965, his reconstructed record of service, shall automatically forfeit his rights or retirement benefits corresponding to his services rendered before June 16, 1951. prll For expediency and in order to avoid the forfeiture clause of the above legal provisions, it is requested that this Circular be read, explained and passed around to all officials and subordinate employees affected. Therefore; All employees, old or new, WHO HAVE NOT YET SUBMITTED TO THE SYSTEM their reconstructed records of service should be enjoined to do so immediately. The GSIS will supply Offices with sufficient copies of GSIS Form No. CR-1 for the purpose. The reconstructed record of service, WHICH JUST BE DULY SIGNED BY THE EMPLOYEE HIMSELF ON THE FIRST LINE IMMEDIATELY FOLLOWING THE LAST ENTRY OF HIS SERVICE, may be submitted to the System ahead of the supporting papers mentioned in the law.
