June 29, 1978 REVENUE MEMORANDUM CIRCULAR NO. 045-78 TO : All Internal Revenue Officers, Employees, and Others Concerned SUBJECT : Loss of One (1) Set of Revenue Tax Receipt Bearing Serial Number 2967411 Notice is hereby given of the loss of one (1) set of Revenue Tax Receipt (CB RCO Form No. 02-02), to wit: Serial Number Quantity 2967411 One (1) set The above Revenue Tax Receipt was reported missing by PASTOR C. REGALADO, Revenue Clerk II, Office of the Collection Branch, Revenue Region No. 4-A, Manila.
