March 27, 2008 REVENUE MEMORANDUM CIRCULAR NO. 035-08 SUBJECT : Lost Two (2) Sets of Unused BIR Form No. 1919 Office Audit-Letter of Authority TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of two (2) unused sets of BIR Form No. 1919 Office Audit Letter of Authority (OLA) with the following serial numbers: OLA199900014906 OLA199900014960 The abovementioned forms were reported as lost by Ms. Emelita M. Braa and Mr. Brian A. Verceles, both of the Assessment Division, Revenue Region No. 16, Cagayan de Oro City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. CTEaDc Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
