TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of one (1) original copy of Tax Credit Certificate (ICC) to wit: TCC No./Serial No. Date of Issue TCC Face Amount 200600003245 March 4, 2011 P11,185,463.62 The above-mentioned TCC was reported as lost by MERCEDES T. CULA, Administrative Staff, LTRAD 3, Bureau of Internal Revenue. Consequently, the entire set bearing the said TCC No. is hereby cancelled. All official transactions involving the use of the said form is therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to undertake necessary measures or actions in order to prevent the improper or fraudulent disposition/use of the same.
