April 2, 2013 REVENUE MEMORANDUM CIRCULAR NO. 032-13 SUBJECT : Lost Fifty (50) Sets of Unused BIR Form No. 2231 Withdrawal Certificate TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of Fifty (50) sets of unused BIR Form No. 2231 Withdrawal Certificate, bearing the following serial numbers: HIACac WCP201100009401-WCP201100009450 The abovementioned forms were reported as lost by Mr. Mariano Sonon, President of MICRO DRAGON PETROLEUM, INC. Bldg. 8008 Leyte Wharf, Cubi Point, Subic Bay Freeport Zone, Olongapo City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
