February 18, 1975 REVENUE MEMORANDUM CIRCULAR NO. 03-75 TO : All Internal Revenue Officers, Employees and Others Concerned SUBJECT : Loss of Twenty-Nine (29) Sets of Unissued Revenue Official Receipts (BIR Form 25.24) Bearing Serial Numbers 1495822 to 1495850; Inclusive Serial Number Quantity 1495822 to 1495850 twenty-nine (29) sets Regional Director Abdon M. Mendigorin, Naga City reported to this Office the loss of one pad of Revenue Official Receipts ( BIR Form 25.24) by ANTONIO B. BORJA, Collection Agent of Calabanga, Camarines Sur. In his affidavit, Collection Agent Borja stated that he went to Naga City on November 8, 1974 bringing with him one pad of official receipts with Serial Numbers 1495801-1495850 for the purpose of collecting taxes when he lost said receipts. He stated further that Nos. 1495801 to 1495821 were already issued and recorded in his previous reports, while Nos. 1495822 to 1495850, a total of twenty-nine (29) sets were still unissued. Internal Revenue Officers, Employees and others concerned are requested to promptly notify this office in case the above-mentioned receipts are found, taking necessary steps in the meantime to prevent the improper and fraudulent disposition or use thereof.
